| Risk Management The shipping industry has always experienced accidents and incidents. Not all have been major disasters, but when one does occur it tends to shake the industry to the core, causing legislation and regulations. People often react as if the disaster was totally unsuspected, and then the investigations and inquiries begin. On many occasions there is surprise that it had not happened before. The risks involved and the error chain that led to the loss of life, explosion or pollution are evident to all, but were not foreseen, nobody had identified the possible outcome from the available evidence.
